Welcome to the EAC FirstStop ‘Home from Hospital Tool’, an online resource designed to inform you of the support and assistance that may be available to you following a stay in hospital.
Our tool is currently still being developed but we are keen to receive feedback on what works and what parts could be improved; this is why you may be asked to fill in a short survey, it will only take a few moments of your time but it is invaluable information to us.
If you feel, having used the tool, that there are services in your area that would be helpful for people being discharged from hospital but we have not included them, then please let us know using the form below so that we can update our database.
