East Renfrewshire's Health and Social Care Partnership (HSCP) fully support the Talking Points personal outcomes approach which is based on years of research on what is important to people who use health and social care services. The starting point for services is to have a conversation with the person to find out what is important to them in life, and then to work out how everybody can work together to achieve the outcomes identified. Sometimes this can involve the person changing or improving some aspect of their life, and sometimes it can mean trying to maintain qualify of life, despite health problems. Focusing on outcomes means trying to ensure the greatest independence and quality of life possible for the individual,
