Who it is for?
Immediate Social Care Limited (I-s Care) is a specialist consultancy and care agency run by a Mental Health, Social Work, which provides advice, guidance and care services in the community for people age 18 and above with mental health difficulties; people with physical disability in need of rehabilitation from hospital, people with long terminal illness, people with sensory impairment, requiring enablement support and people who suffer with dementia.

What to expect then:
Once Immediate Social Care Limited is contacted before service commence a qualified Social Worker will visit the customer to assess his or her needs. Formulate a personalized care plan and risk assessment. With the consent of our customer, we will also make contact with any other professional involved in the client care including GP's and advocate on customers behalfs.
