First Contact referral scheme for people in South GloucestershireWhat does it do?
The First Contact Scheme supports older people in South Gloucestershire to access services to which they are entitled. This is achieved by maximising the opportunities for agencies routinely visiting or coming into contact with older people to complete a simple checklist to identify if a person will benefit from the services offered. The referral will be passed to the relevant agency to contact the older person directly and to deliver the appropriate services.
It encourages take up of services through trusted and known professionals and volunteers who can explain the positive side of seeking help and the types of service available. The scheme uses a simple, quick and easy checklist of 12 questions, enabling staff and volunteers to ascertain the needs of the person they are dealing with. The checklist is completed by the agency that first has contact with the older person; it is ususally completed by the member of staff or volunteer from the agency in conjunction with the older person. The checklist is available in South Gloucestershire libraries and at events for older people to
either complete themselves or complete with the help of staff.
