Active Dementia Support
What does it do?
As your Activity Support Worker, I will help plan and facilitate a schedule of activities and pastimes that are meaningful and pleasurable for the person with dementia. This plan can change with the condition. Activities may be at home or in the community and are always one to one. Carers can join in or take a break- knowing that the person they care for is doing what they want, when and where they want to be doing it.
Who it is for?
People with dementia and their family carers in parts of East and West Sussex.
Where it is available?
Local service serving within 24km of BN108BB.
What it costs:
£15 per hour (minimum of 2 hour sessions). Full and half day sessions are also available. Bank Holiday Mondays are charged at £22.50. Travel up to 10 miles to and from an activity is included in the hourly fee. Additional mileage is charged at 40p per mile.
