What does it do?

The Lifestyle Support service provides help for people who are very
isolated because of a serious physical or sensory disability or medical
condition.

They may find it very difficult, or impossible, to
take part in ordinary activities that involve going to places and
meeting people.

The service will provide individual help to a
person to help them overcome the barriers that are preventing them from
living the life they would choose. If a person has a carer, the service
will listen to their needs and support them.

Who it is for?

Lifestyle Support is for adults of any age who are experiencing social
isolation and who have a disability or medical condition which seriously
limits their ability to live the sort of life that they would wish for.
They should be assessed by social care staff as requiring this service.

Where it is available?

Local service (Mendip, South Somerset, Taunton Deane, West Somerset, Sedgemoor).

What it costs:

There is normally a cost for some parts of this service. You can contact us for details of individual charges for activites.
