What does it do?
An assessment is simply a way of finding out the best way of helping you to cope.
Who it is for?
Cheshire West and Chester has a duty to provide and co-ordinate the assessment of those people in its area who fit the legal definition of a “Carer”. You may choose to have a separate assessment if you wish or a joint assessment if that is what you and the person you care for prefer. If you choose the latter you may request at any time in the future to have your needs assessed separately.
Where it is available?
Local service (Ellesmere Port and Neston, Vale Royal, Chester).
What it costs:
There is no charge for an Assessment.
How to access or apply for it:
For further information call 0300 123 8 123.
Any help provided will be reviewed periodically to ensure it is meeting your needs and those of the person for whom you care. If circumstances change, you can request a review or reassessment at any time by contacting your named member of staff or the Access Team.
