Hospital Discharge Service
What does it do?
The Hospital Discharge Service provides short-term flexible support to older people on their return home from hospital. The paid workers who visit you are called Settlement Aides. They are trained staff who will be able to assist you with most day to day tasks such as shopping, preparing a light meal, paying bills, collecting pensions, prescriptions etc. The Settlement Aide can also spend time sitting and chatting over a cup of tea if there's not many practical things to do. The Settlement Aide will also be able to contact other services on your behalf.
As long as we have sufficient notice, the Settlement Aide can visit you on the day you are discharged home. We usually make 3 -4 visits, these can be on consecutive days or spread over a period of weeks. The Settlement Aide will agree your plan of visits with you. It is important to remember that the service is only short-term (on average 3 or 4 visits).

Who it is for?
Older people on their return home from hospital. Anyone over the age of 60 years living in Cardiff and the Vale of Glamorgan can receive our service.

What to expect then:
Our service operates seven days a week 9am -5pm.
An evening service is available Mon -Fri subject to the availability of staff and the demand for the service.
Our involvement does not affect your rights and access to any other services.

Our team of Placement Advisors based at Cleeve House, support older people to make an informed choice when moving from hospital to a care home. Referrals to the service should be made by a social worker or nurse.
The Hospital Discharge Service is registered as a Domiciliary Care Agency with the Care and Social Services Inspectorate Wales.
