Ultimate Independence Care is a CQC-registered care provider that delivers high-quality, person-centred support to individuals across the South West of England, including Bristol, Gloucestershire, South Gloucestershire, Somerset, Wiltshire, and surrounding areas.

Our service is designed to promote independence, dignity, and choice. We support older people, individuals with learning disabilities, mental health needs, physical disabilities, and those with complex support requirements to live as independently as possible in their own homes or supported living settings.

We offer a wide range of services including:

  • Domiciliary (home) care

  • Supported living and community outreach

  • Live-in care

  • Respite and transitional support

  • 24-hour and waking night care

  • Personal care and medication management

  • Support with daily living, mobility, and companionship

  • Support with accessing the community, appointments, and meaningful activities

 

All support is tailored to individual needs, goals, and preferences. We work closely with families, local authorities, social workers, and other professionals to ensure a consistent and responsive approach that enhances quality of life and wellbeing.

Who it is for?

Our service is for individuals aged 18 and over who require support to live independently, either in their own homes or within a supported living environment. We are registered to provide care across England, with a strong focus on delivering services throughout the South West region.

We primarily support:

  • Older adults who may need personal care, assistance with daily living tasks, companionship, or help managing health conditions and mobility.

  • Adults with mild to moderate learning disabilities, supporting them to develop life skills, engage with their communities, and increase independence.

  • Individuals with mental health needs, including those recovering from crisis or requiring ongoing emotional and psychological support.

  • People with physical disabilities who may benefit from support with mobility, personal care, or adapted accommodation.

  • Those transitioning from institutional care—including hospitals, residential homes, or secure settings—into more independent living, including individuals who require specialist community-based support.

 

Our services are inclusive, person-centred, and tailored to each individual’s needs, goals, and lifestyle. While we are able to support placements across England, our operational hubs and current service users are based mainly in Bristol, Gloucestershire, South Gloucestershire, Wiltshire, Somerset, and surrounding areas of the South West.

Regional service (Berkshire, Bristol, Devon, Gloucestershire, Greater Manchester, Herefordshire, Oxfordshire, Somerset, Warwickshire, West Midlands, Wiltshire, Worcestershire).

What it costs:

Our service costs vary depending on the level and type of support required, the frequency of visits, and whether care is delivered in a supported living setting or in an individual's own home.

We work with both self-funded clients and those supported through:

  • Local Authority adult social care funding

  • Continuing Healthcare (CHC) funding

  • Direct Payments or Personal Budgets

  • The Ministry of Justice or other statutory agencies

We are happy to provide a free, no-obligation care assessment to understand each individual's needs and provide a clear cost estimate based on their specific support plan.

Where help is available, we assist individuals and families in navigating funding options, including applying for local authority assessments, benefits (such as Attendance Allowance or Personal Independence Payment), and ensuring access to the right funding stream for their situation.

  2. Professional referral: We accept referrals from local authorities, NHS teams, hospital discharge coordinators, social workers, case managers, and other professionals.

  3. Online Enquiry: Visit our website to complete a simple online referral or contact form. One of our team members will get in touch promptly to arrange a suitable follow-up.

  4. In-person or telephone assessment: Once we receive an enquiry, we offer a free consultation or assessment to understand the person’s needs, preferences, goals, and living situation. This helps us build a tailored support plan.

We aim to respond quickly and support individuals and families through every step — whether they are privately funded or applying for support through a local authority, direct payments, or NHS Continuing Healthcare.

What to expect then:

Once an enquiry or application is received, applicants can expect a smooth, supportive process:

  1. Initial Contact: A member of our team will get in touch to discuss the enquiry, gather some basic information, and explain our service in more detail.

  2. Needs Assessment: We will arrange a free, no-obligation assessment — either in person, by phone, or virtually — to understand the individual’s needs, preferences, goals, and daily routines.

  3. Support Plan Proposal: Based on the assessment, we’ll create a personalised support plan and proposal. This will include recommended hours, type of care, and any specific accommodations or considerations.

  4. Funding & Agreements: We’ll support the applicant to explore available funding options if needed (e.g., local authority, direct payments, CHC, or private funding). Once agreed, we finalise service agreements and schedules.

  5. Matching and Introductions: We carefully match the individual with trained support staff. Where possible, we offer introductory visits to help build rapport and confidence.

  6. Service Start: Support begins as agreed, with regular reviews to ensure satisfaction, progress, and ongoing suitability.

We stay in close contact throughout, offering guidance and reassurance at every step to ensure a positive, person-centred transition into support.
