What to expect then:

During Your Call

This is our first point of contact. Over the phone, or in person, we can learn a bit about you, your circumstances, and the impact your condition has on your daily life.

It is also an opportunity to discover more about the service we offer and how we may be able to support you.

Assessment

Your care needs assessment will be conducted by Martha in your home at a date and time of your choosing. This way she can be in the environment where care will be provided, and it gives you the opportunity to point out any specific challenges you are facing at home. The assessment also helps us to assess any risks of harm that may come to you or your carer.

You will be asked a series of questions which, at first, seem a bit intrusive and unecessary, however, it gives us enough detail about you and your circumstances to ensure we are able to support all your needs. ?We may ask details such as what/who is important to you, your interests, likes and dislikes, and your preferred routines.

Most importantly, as a service, the assessment helps us to learn more about what you would like to achieve with your care and support, your goals and your aspirations for the future.

*Please note. If it is an urgent care request the process is much more condensed, focusing on the vital and essential information we need to ensure you are safe, your needs are met, and your wishes are respected.

Planning Your Care

Using all the information you provide us during the assessment, we will create your individualised care plan. It will clearly outline how your care and support needs to be delivered, including:

  • Your choices and preferences

  • Desired outcomes

  • Essential information for use in case of emergencies

  • The roles and responsibilities of everyone involved in your care; and

  • any associated benefits and risks the plan poses to your health and wellbeing.

Start Care

Once you are ready to receive care you will tell us when you want to start. On your first visit a member of our management team will attend with your carer to introduce you to each other and to familiarise them with your personal needs, choices and environment.

It is an opportunity for you to become aquainted and learn about each other.
