Age UK Exeter have co-ordinators working in The Royal Devon University Healthcare Foundation Trust, as part of the Acute Care of the Elderly (ACE) Team currently based on Capener ward. Their role is to offer support to older people who have had a short stay in hospital to access a variety of community services.

Patients or their families often need additional support to ensure suitable care is in place when returning home, our co ordinators will look to provide information & advice as to where support can be found, whatever the situation.
