Your local NHS in Kensington & Chelsea has recently introduced a new integrated care service for older adults called My Care, My Way. The service is designed to provide you with additional support to help you manage your health and wellbeing, prevent unplanned hospital admissions and help you to stay independent.

Managing your health and care can be confusing when there are lots of different people and organisations involved, for example GPs, social workers, hospitals, community organisations and charities such as Age UK K&C.
