What does it do?
We commission services for older people to help them continue to live independently in their own home wherever possible. Some of the services we commission include:
• Social care assessments
• Personal or domestic help to stay at home
• Meals delivered to your home
• Day services and activities
• Carers services, for example, short breaks and leisure and learning opportunities
• Reablement services to help you home from hospital or to prevent admission
• Advice, equipment and adaptations to support independent living
• Residential, nursing and extra-care placements
• Housing related support services
• Other services including lunch clubs, drop-ins and support in your community
• Information and advice about other services
