Hospital services scheme for older people in York

What does it do?

Our hospital services scheme is a flexible service of up to six weeks, to support you when you leave hospital or intermediate care. We can provide help with shopping, including on your day of discharge, take you for a short outing (where appropriate), call in for a chat, ensure that you have the services and equipment you require, and generally check that you are recovering well. 

We can also provide volunteers to sleep overnight in your home for up to three nights, to provide you with company and companionship, and help you feel confident in your own home after a spell in hospital or intermediate care.

 

Who it is for?

We help anyone over the age of 60 who lives in the  City of York Area and needs support on discharge from hospital or intermediate care, including the day units and endoscopy.
