Home from Hospital service for older people in Medway

What does it do?

The service provides support for people who have recently been discharged from hospital, and aims to reduce the risk of being quickly re-admitted. The home from hospital service has recently moved and is now operating from our main offices at Chatham's Historic Dockyard.

This service is available for up to six weeks at two hours per week and it aims to build confidence, reduuce isolation and enhance wellbeing. We appreciate that everyone is different and has varied needs, this is why we will always aim to deliver a tailor-made support service delivered by our volunteers FREEof charge.

Home from Hospital is a free service for people living in Medway Area. The service provides short term help and support for people over the age of 50.
