Home from Hospital service for older people in Worcestershire

What does it do?

Are you worried about coping at home after a stay in hospital?

Our Home from Hospital service provides trained volunteers to help older people regain their independence and confidence for up to six weeks following a stay in hospital. The help is flexible, based around what you need and may include:

Shopping – One-off cleaning to enable your safe return home – Collecting prescriptions – Support to attend healthcare appointments – Emotional support – Signposting to other services. 

It does not include any type of personal care.

Who it is for?

The service is available for people aged over 55, who are resident in Worcestershire and who are unable to cope independently in the short term or who do not have other home support from family or friends.

Where it is available?

Local service (Malvern Hills, Worcester, Wychavon, Wyre Forest, Bromsgrove, Redditch).

What it costs:

There is no charge for this service, which is fully funded by Worcestershire County Council.
