The aim of the Home from Hospital service is to prevent hospital readmission and premature admission to residential care. We can assist you with the support you need to keep your independence and help you regain your confidence after your discharge from hospital.

You can access this FREE service within 7 days of discharge. You can refer yourself, or ask the hospital or doctor to do this on your behalf. It doesn't matter if you live with someone. We can support you for about 4 weeks.
