The aim of the Home from Hospital service is prevent re-admission and premature admission to residential care. We can provide you with all the necessary support required to keep you independent and help you regain confidence after your discharge from hospital.

Working directly with you we are able to tailor this service to suit your personal situation.We can make sure your home is clean and warm, fill your cupboards with all the essentials and even collect a pet in time for your arrival home. From here we can signpost you to beneficial services and groups to help you regain confidence and feel more settled.

"Adults aged 18 years and over who need assistance to regain their independence after a spell in hospital and who need short term support"

Volunteers are vital to the Home from Hospital service, whether to act as a Telephone Befrienders or as a friendly face to welcome someone home and visit for a short time. In either of these capacities you will be invaluable in providing comfort and support to service users in those vital first weeks.
