Home from Hospital Service in Manchester

What does it do?

Manchester Care & Repair, the City Council and NHS Manchester are working together to help older people across the City.
If you’ve been ill, you may appreciate extra support with things like increasing your welfare benefits, keeping warm at home and day-to-day tasks.

If you are admitted to a Manchester hospital for any reason, we will call you when you are back at home to see how you are doing. Don’t worry, if you wish to you can opt out of this service at any time.

We will not be given any medical information, only the information we need to be able to contact you. We store all information securely and it will not be shared with anyone else without your consent.

Care & Repair can ring you after you leave the hospital and offer:

  • A free Handyperson Service – for small practical tasks
  • A home visit from a trusted caseworker for advice and support with services or welfare benefitsAdvice on home safety and falls prevention
  • A helping hand – with immediate needs e.g. outstanding bills, shopping, buying equipment (we don’t do personal care or household cleaning but can direct you to these services)

Over 60's.
