Hospital and housing discharge serviceWhat does it do?
Care & Repair Worcestershire have two Housing Discharge Coordinators based within the Integrated Discharge Teams at The Worcestershire Royal Hospital in Worcester and at The Alexandra Hospital in Redditch.
The Housing Discharge Coordinators have built close relationships with local hospital staff, and Social Services. The service aims to provide practical housing advice for people entering and leaving hospital. This reduces 'bed blocking' and enables people to leave hospital when ready, returning to their own home, or to somewhere more suitable of their choice.
The Housing Discharge Coordinators can:
• Give advice about housing options
• Arrange for aids and adaptations to your own home
• Help you to access welfare benefits
• Organise homecare and handyperson services
• Link to befriending initiatives and community alarm services.
The Hospital Discharge Service improves the quality of life for people and enables them to remain in their own home if that is their choice; it reduces 'delayed discharge' fines for Social Services, levied if people are not able to leave hospital when ready; it frees up hospital beds and reduces re-admissions, and referrals to residential care.
