Each client has a dedicated named care manager or team leader, so they always know who to speak to for direct answers.
Clients care is provided by a team of qualified and professionally competent care workers. Each carer has been vetted by the Disclosure and Barring Service (formally CRB). Carers are carefully matched with clients to ensure compatibility.
Care managers, trained to the governments "personalisation agenda" meet and work alongside new clients before they develop the client's care plan.
Person Centred care plans are compiled from information from the clients; professionals who are already involved in the clients social and health needs; and family members and friends.
We write a "Profile Page" for each customer. This helps ensure a person centered approach. The profile Page lists, among other things, individuals' tastes and preferences.
There are plenty of opportunities for face-to-face meetings between the care manager, family members and other experts to discuss and review a clients expected outcomes. Clients are regularly encouraged to contribute to their care plans.
Care managers can also advise, make recommendations and sign post clients to services that would be of benefit to them. We do all we can to promote independence.
Weekly service sheets are provided to each client, detailing which carer is coming and at what time.
Clients are notified of any unavoidable changes to their schedule by telephone.
Electronic monitoring is available at no extra cost, to eliminate the risk of a service being missed and to ensure carers provide their paid for time.
Priory Homecare Ltd operate a 24 hour help line, which means support is always available for any crisis or problem.
