The Access Team is the first point of contact and access for the general public, existing service users, carers, third parties, health and social care professional and other agencies to the Adult Care Service.
The team has a key role within Social Services in dealing with enquiries, giving information or advice, sign posting to other
organisations or directorates and taking referrals for community services.
Services provided can include:
• healthcare
• equipment
• help in your home or in a care home
• community support and activities
• day centres
• help for people with disabilities
• financial support
• information and advisory services, and advocacy
• support for carers
