What does it do?


Specifically we offer social work support in the following areas:
  • Community care assessments of need
  • Direct payments and personalised support planning
  • Reviews of care packages
  • Advice on residential and nursing placements
  • Carers assessments, advice and support.
  • A Multi agency Safeguarding approach with adults deemed at high risk

Older people and adults with physical disabilities
