What does it do?

The Planned Care Group provides a range of dementia services to people living in Northumberland, Tyne and Wear.  They are:

Assessment, Treatment and Rehabilitation Day Hospital

Location: Castleside Day Unit, Centre for the Health of the Elderly, Campus for Ageing & Vitality, Westgate Road, Newcastle upon Tyne, NE4 6BE
Description:
Assessment, treatment and rehabilitation by a multi disciplinary health and social care team for older people with mental health problems arising from functional or organic disorders.
Day Hospital also have Memory Clinics in Newcastle upon Tyne Includes memory management services providing assessment, treatment, diagnosis and monitoring of medication as per shared care protocols

How to get referred: Via Specialist Mental Health Service accessed via GPs

Location: Cresswell Ward (24 beds), St George’s Park, Morpeth, Northumberland, NE61 2NU
Description: Assessment, treatment and rehabilitation by a multi disciplinary health and social care team for older people with mental health problems arising from organic disorders such as dementia

Intermediate Care
Location:
Centre for the Health of the Elderly, Campus for Ageing & Vitality, Westgate Road, Newcastle upon Tyne, NE4 6BE
Description: Community psychiatric nurse’s provide support to 10 community rehabilitation beds in Chirton House Local Authority Resource Centre and 10 community rehabilitation beds in Connie Lewcock Local Authority Resource Centre and into the community resource team across the city. 
Intermediate Care also provide support to Byker Bridge
