Chiltern Older People's Community Mental Health

What does it do?

Chiltern Older People's Community Mental Health Team is a
multi-disciplinary team providing services to older adults who suffer
from severe and enduring complex mental health difficulties.

We liaise directly with primary care to offer a seamless service for
those in need of more specialist services.

  • Older adults (over 65 years)

How to access or apply for it:

Referral to this service is through the GP.

Office hours: Monday to Friday 09:00 - 17:00

The team has no facilities for visitors. However, the team can arrange
meetings in alternative rooms in the hospital if necessary.
