Community Services

What does it do?

Home from Hospital
Through this service we provide short-term practical help and support to older patients on being discharged from hospital. Dedicated volunteers provide practical help, such as shopping, collecting prescriptions or accompanying frailer patients to appointments. The service also provides support and reassurance to clients, as well as providing information and assistance to access other services.

Befriending and Forget-me-Not
One of our most important services which provides long-term support for lonely, frail or housebound older people who are living alone and do not have family or friends living nearby. The essence of the service is regular support and friendship which is delivered by a weekly phone call or by a regular volunteer visitor to share a cup of tea and a chat.
