What happens next?
We will make an appointment to visit you at home and carry out an assessment. If we’re unable to offer you day care at the Sundial, we will provide information on other organisations that may be able to help you.
We will also invite you on a trial visit, so you can check it suits your needs.
After you've been offered a day care place
A member of staff will visit you at home to discuss the arrangements. Things like the number of days you attend, what time to pick you up, whether we need keys to your home, and any dietary requirements.
