Day Care

What does it do?

Day Care and day opportunities is offered to people who need some help with personal or practical care and are unable to get out and about by themselves.

Provided in day centres or community buildings around the borough, they offer an opportunity to take part in a wide range of social, leisure, training and educational activities. Caring staff ensure personal care needs are met with dignity.

These services aim to improve the quality of people's lives, promote independence and support people to remain in their own homes.

See list of services at http://www.lewishammylifemychoice.org.uk/day-care.aspx

Who it is for?

People who live in the borough of Lewisham, who are elderly, or over the age of 18 and have learning or physical disabilities, sight or hearing loss or who experience dementia.

Where it is available?

Local service (Lewisham).

What it costs:

There will be some cost but the actual amount you pay will depend on how often you use day care.

Self Directed Support gives you more choice and control over the services you receive. This means that following a social care assessment you will be given a personal budget from us. You can choose to use it to arrange your own support and look after the money yourself or you can ask the Council to hold the money and help you find the support you need.

How to access or apply for it:

The Adult Social Care Advice and Information Team is the first point of contact for all referrals and general enquiries. You can contact them from 9am to 5m, Monday to Friday on 020 8314 7777. In an emergency out of normal office hours contact the Out of Hours service on 020 8314 6000.

When you contact us we'll discuss your situation over the phone, check that we've got all our facts straight and then follow this up with a visit to your home to talk about your personal and practical needs and to discuss what help is available.

What to expect then:

In order to find out just how much help is needed to make life easier for you, we do need to carry out an assessment.

After the assessment we will identify and agree your needs with you and discuss the help and support available to meet them, according to the seriousness of your situation.

What is an assessment? See Lewisham's website at http://www.lewishammylifemychoice.org.uk/what-is-an-assessment.aspx
