Carers Assessment

What does it do?

A Carers Assessment is simply a discussion with you that will help us understand the physical, emotional and practical impact that caring has on your life and to ensure that your needs are taken into account.
As a carer you have a right to have your needs considered when decisions are being made about support for the person you care for. This will include any education, training, work or leisure activity that you would like to take part in.

Who it is for?

If you are an adult providing a regular and substantial amount of unpaid care for someone aged 18 or over, you can ask for a carers assessment.

Where it is available?

Local service (Trafford).

What it costs:

There is no charge for a Carers Assessment.

How to access or apply for it:

The Community Screening Team is the first point of contact for all referrals and general enquiries. You can contact them from 8.30am to 4.30pm, Monday to Friday on 0161 912 5199. Textphone 0161 912 5129.

Additional information:

When you contact us we'll discuss your situation over the phone. Depending on your personal situation, we may follow this up with a visit to your home to talk about your needs, discuss what help is available and what type of home would best meet your needs.
