South East Community Mental Health Team
What does it do?
The South East Community Mental Health Team (CMHT) provides a service to older adults with mental health problems from the age of 65 upwards; and to younger service users with a diagnosis of early on-set dementia, in the south east of Hillingdon. Specailist assessments and a variety of therapeutic interventions are delivered in people's homes and local outreach facilities. The team has close working relationships with social services, and voluntary and statutory services.
The team comprises a team leader, a consultant psychiatrist, an occupational therapist, a psychologist, a social worker, a community mental health nurse and a support worker who give varying amounts of time to the team.
