Memory Clinic - Hammersmith & Fulham

What does it do?

The Hammersmith and Fulham Memory Disorders Clinic is a specialist service that provides assessment, treatment and support for people
experiencing memory problems and other related cognitive difficulties. Following referral to our services (usually by your GP), you will be seen by our Memory Triage Service within your own home. This means you assessment takes place in familiar surroundings and helps
us to determine whether your follow-up assessment would be better placed with the hospital or community based memory clinic.

People with dementia in the Hammersmith and Fulham areas.

What to expect then:

The hospital based Memory Clinic aims to provide a ‘one-stop’ service where all important assessments and investigations can be performed on a single day. You’ll be met by an administrator who will outline your own specific timetable for the day. This will include an assessment by one of our consultants which will last up to an hour.
