What does it do?
A Diagnostic Assessment Clinic which offers clinical assessments of levels of cognitive impairments, diagnosis, reviews and medication in some cases. All assessments are carried out at the clinic; however flexibility is applied in cases whereby home visits are required.

This service is for adults with cognitive impairment or memory difficulties within City & Hackney.

The service accepts referrals from GPs and other Community Health Care for Older People profesionals.
