Support for Carers
What does it do?
The carer's assessment is for you, the person you care for does not have to be assessed as well. You can choose to be assessed together.
The assessment covers:
- how many hours of care you’re providing and what type of care
- whether you have any free time to yourself
- if caring is affecting your health
- practical help you might need with caring, or with household tasks
- whether you can take a break each year
- whether you would like to work, do a course, or have a hobby
- what you would like to happen if you are unable to look after the person you care for in an emergency
Someone will come out to see you but if you wish you can begin to fill in the assessment form which you can download below. If you wish to complete the assessment yourself please send the completed form to your local social care office and someone will contact you.
