Puddletown Neighbourcar Scheme
What does it do?
A Voluntary Car Scheme providing transport for patients who are unable to drive and who have no other means of getting to medically-related appointments, ie hospital and visits to other health professionals such as dentist, physiotherapist etc.
Who it is for?
Patients of Puddletown Surgery.
Where it is available?
Local service (West Dorset).
What it costs:
Dorset Partnership for Older People (Dorset POPP) provided start-up funding for this scheme, with the aim that the scheme would become self-funding. There is a joining fee of £5 and you will be charged mileage at the rate of 50p per mile for your journey.
How to access or apply for it:
To register, either telephone Mike Chaney on 01305 848352 or pick up an application form from Reception at the surgery.
What to expect then:
When your application form has been received, you will be contacted with details of the telephone number to ring for bookings.
Additional information:
The scheme operates Monday to Friday. Our drivers are volunteers and use their own vehicles. Please give as much notice as possible.
