Memory Service (Lambeth and Southwark)

What does it do?

The Southwark and Lambeth Integrated Memory Service (SLIMS) offers comprehensive assessment, treatment and support options to anyone over the age of 18 years who is experiencing mild to moderate memory problems in the boroughs of Lambeth or Southwark.
 
The service is comprised of a multidisciplinary team who work in collaboration with the Alzheimer’s Society.
 
We offer a comprehensive assessment package based on the principles of recovery, and we are committed to supporting carers of people with dementia.
 
Following assessment, we coordinate individualised care planning. This may include prescribing of medication if appropriate, post diagnostic support and signposting, problem solving strategies, or individual or group therapy which is offered for both clients and carers. We can also organise for continuous follow up though our dementia advisors. We have a close working relationship with the memory clinics at St Thomas', Kings College and Guys hospitals to ensure we provided the highest quality specialist care.

People who have memory problems that may be associated with dementia. We care for people who live in the London boroughs of Lambeth or Southwark.
