The service offers practical support such as help with shopping, cleaning, GP appointments and medication ordering. The service lasts for 2-3 weeks.

Our experienced staff work with you to ensure that you are settled in at home as soon as possible after being discharged from hospital, they can also support with information, advice and signposting to other services that will help you in the future.
