Care Direct - Help When You Leave Hospital
What does it do?
They can support you to live as independently as you can. This can be done by working with you and people involved in your care to:
- plan for you to leave hospital safely
- help you get back to independence quickly
- help arrange the care you need when you go home
Who it is for?
The Hospital Social Work Team can help if you are being discharged from hospital and need care.
Where it is available?
Local service (Bournemouth).
What it costs:
There is no charge for assessing your needs and giving advice and information. However if you need support services after you leave hospital, you may be asked to pay for them. How much you pay will depend on your finances.
How to access or apply for it:
Nursing staff will refer you to the Hospital Social Work Team if:
- you will shortly be leaving hospital and
- you are likely to need care after you leave
- you agree to someone from our team.
What to expect then:
An assessment of needs will be carried out where you can discuss what you can do for yourself and what support you need to be independent.
Intermediate /after hospital care, Advocacy, Support for carers / Relief Care / Respite Care, Home from hospital services, Personal care at home (registered)
