Reablement & Assessment Service
What does it do?
This service is provided by care staff that support you in your own home to:
- Relearn your skills and confidence in completing everyday tasks and activities; this could be after leaving hospital, after an illness or a gradual deterioration in your general health or mobility.
- Support you to gain new skills to help you manage at home.
- Help avoid going to hospital, long term residential care or longer-term dependence on services.
- Help you to carry out exercise or other therapy prescribed to you.
Who it is for?
Re-ablement is about giving you, as an adult, the opportunity and confidence to relearn some of the skills you may have lost because of poor health, disability or after a stay in hospital.
