Intermediate support at home
What does it do?
Intermediate Care Services have been developed in Bracknell Forest primarily to support and enable people to remain in their own homes for longer and to facilitate timely discharge from hospital back to individuals' usual places of residence. The aim of the service is to:
- Promote independence and maintain individuals in the community by providing short-term rehabilitation programmes to maximise individual potential
- Prevent inappropriate hospital admission and re-admission
- Facilitate timely, safe and effective discharge from Acute Care
- Maximise individuals’ independence and thereby reduce the need for long term care and support
- To reduce the need for long term social support and minimise the need for long term placements
- To contribute to a reduction in the number of individuals falling by providing comprehensive assessment and appropriate referrals
