What does it do?

Transform Homecare is a service provided by Transform Housing & Support. We have operated a homecare service since 2005. We currently provide care for more than 130 people across Surrey and parts of West Sussex. We give care to help people live as independently as possible in their home.
Our team is well trained, experienced and highly professional.
Our service starts with a thorough, professional assessment of the customers needs. We will agree with them, and anyone else they wish to have involved, a personalised support plan which accommodates all their needs. Our service is flexible and responsive, so if those needs change even for a short period, for example following a stay in hospital, we can increase the care package for as long as required.
Our purpose and values are at the heart of what we do.
We put the customer at the heart of our range of services which include:
companionship and emotional support
personal care including washing and dressing
help getting up and going to bed
support with taking prescribed medicines
attending medical or other appointments
meal preparation
light housework including tidying, cleaning and ironing
involvement in the local community.
We can also help by providing information and advice on benefits, healthy living and keeping well. Whatever support a client needs there’s a good chance we can help to provide it, or will know someone else who can. 

Who it is for?

We provide person-centred homecare services to help people to live independently and with dignity. 
Transform Homecare is a not for profit, care and support organisation. This means that all income is returned to the service. We have been providing care for customers in their own homes for almost 15 years, having developed originally to promote greater exclusivity and help create stronger communities around Surrey. Transform has come to be one of the leading providers in the region.
Our clients may need help with personal care, managing money or assistance in finding new friends or accessing activities they would like to do.  
We offer care and support to those with a range of needs including
Older people who need support to live independently
People with dementia
Adults with learning disabilities
People with physical or mental disabilities

Regional service (Surrey, West Sussex).

What it costs:

When you are choosing a care provider, it is important to think about how your care will be funded.
There are several different ways to pay for care services:
You may be eligible for local authority funding, depending on your income and savings
You could pay for your care yourself
Your family member may agree to pay for care
You might be able to do a combination of these

How to access or apply for it:

Personal budget - This is a government initiative which allows individuals to purchase their own services. To find out if you are eligible, please visit the NHS Choices website.
Privately funded - If you choose to pay privately, contact us today to set up a meeting to discuss your individual needs.  We will come and meet with you and your family if you wish to discuss your requirements.

What to expect then:

Once you've contacted us and we've discussed the clients initial needs, we will make an appointment to come and meet the client and anyone else that needs to be involved.
During this meeting we will go through some forms to discuss the clients requirements, as well as discuss when the package of care can commence.
