What does it do?

Our care at home service offers you the choice to remain independent in the comfort of familiar surroundings, therefore allowing family and friends to be involved with your care at all stages, without compromising on the quality of care.

We understand that sometimes decisions need to be made when caring for the ones close to us. These decisions need to be made from an informed position and so our team will work with you to ensure that the best care package is put into place to meet needs.

We pride ourselves on continuity of care which means you have regular staff who know you and your needs well. Our friendly staff will talk you through the process of booking a healthcare professional and then tailoring the care package around the needs of the end user and the family.

What is included?
-A care package designed for you that is supported by your family and friends
-A detailed initial assessment with one of our team who will offer you some possible solutions to your care need requirements

-Access to support 24 hours a day

The Carebridge team will visit you and carry out a full assessment of your care needs. This will give us an opportunity to get to know you and understand any personal preferences that you may have.

To gain a full understanding of your care requirements and to ensure that all aspects of your care are arranged correctly, we also spend time liaising with other professionals involved in your care as well as family.

All our clinical care packages are managed by an experienced nurse who will ensure that your care service is regularly reviewed and managed effectively.

We are open 24 hours per day, 7 days per week, so if you have any concerns at any time of the day there will always be someone that you can speak to.

We are registered with Care Quality Commission (CQC) and the UKHCA and we ensure all our care consistently meets and exceeds the required levels of care delivery.

Carebridge is an independent boutique care service that delivers support to people in their own homes. We offer a range of services from companionship to complex care and work with our clients to ensure that the correct level of care is in place and consistently delivered. Carebridge is a specialist provider of private nursing, PHB, case management provision, home from hospital, CHC and domiciliary services direct to private individuals suffering from chronic, long term or complex conditions for adults and children.

We believe high quality home care services should be accessible to all people. Regardless of the level of your need or budget, we will strive to deliver to you the best service possible.

Carebridge home care help is available as four hourly slots, daily, holiday respite, and full-time live-in care options, and we’re flexible and open when it comes to making changes. It’s our priority to see that your care works for you. At Carebridge no two care packages are the same. Just let us know what you want and we will deliver it.

For some of our clients, the Carebridge model of care means reassurance and companionship for a few hours a day. For others, it means 24-hour live-in care, with complex lifestyle assistance and the attention of an expert team of carers, managers and clinical advisors.

Each package of care is assessed on its own merits, part of the care needs assessment will look at the finance side and we will let you know of the options available to you.
