This free service is based at the Royal Bolton Hospital and provides up to six weeks support following a recent discharge from hospital or A&E. It is specially directed towards those aged 65 and older who live alone, or a main carer.

Developed in partnership with Bolton NHS Foundation Trust Hospital and Senior Solutions, the service is aimed at supporting older people make a safe discharge from hospital and provide them with the practical and emotional support they need to recover at home.
