People can access or apply for service by calling or emailing
What to expect then:
Applicant should expect in less than 24 hours referral and information visit booked with applicant, followed by unique support plan for tailored services for clients
Additional information:
The service will match each client with a Lifestyle support officer to work through individual plans for client aiming at the expected outcomes to live well and remain independent in their homes.
