This is a free service for older people and their carers to enable possible early discharge from hospital to their home. It then offers support, information and assistance to people who use the service and for carers of people who have had a stay in hospital.

The service is provided by fully trained staff who will offer a non clinical assessment of needs and will complement any health or community based provision, ensuring a holistic approach to all physical and emotional needs helping older people to adjust to any changes following a hospital admission.
