What does it do?

Age Concern Hampshire, in partnership with Southern Health NHS Foundation Trust, are providing a new person-centered support service to patients on rehabilitation Wards at community hospitals in Gosport and Petersfield. Designed to promote mobility, dexterity, social interaction, cognitive stimulation and nourishment, a range of activities are hosted by our Health & Wellbeing Co-ordinators, with the support of a team of Volunteers. Further support is provided on leaving the hospital with links to our home visiting support and befriending services to ensure clients have all the support they need.

Close collaboration with NHS professionals including Ward Staff, Physiotherapists and Occupational Therapists, and liaison with Hampshire Adult Services & other partner organisations, ensure that patients have been given all necessary support and consideration to make the transition home a smooth as possible and avoid future readmission where possible.

Age Concern Hampshire are working in partnership with the NHS to improve health & wellbeing for patients whilst staying in hospital.

Our Hospital-to-Home Service is currently based at Alton, Gosport and Petersfield Community Hospitals.

Where it is available?

Local service (East Hampshire, Eastleigh, Fareham, Gosport, Hart, Havant, Southampton, Test Valley, Winchester, Basingstoke and Deane, New Forest, Portsmouth, Rushmoor).
