Hospice at Home Service

What does it do?

We bring the specialist skills of Hospice staff into your home, providing practical hands-on nursing care and access to telephone advice 24 hours a day, 365 days a year.

Who it is for?

If you would prefer to receive your care within the comfort and familiarity of your own home our Hospice at Home service is here to help.

Where it is available?

Local service (Eastbourne, Hastings, Lewes, Wealden, Brighton & Hove, Rother).

What it costs:

These services are provided free of charge.

How to access or apply for it:

If you think you could benefit from the services of Hospice at Home please do contact us.  You can also discuss your referral with your GP or District Nurse.
Referrals are also made by Macmillan Nurses, hospital consultants, hospital doctors, nurses and all allied health professionals including nursing and care homes.

What to expect then:

A trained nurse will always carry out your first assessment.

Our service is nurse led ie. whatever the service provided, it is the responsibility of the nurse who instigated it.  Tasks will only be delegated to a Nursing Auxiliary if your nurse is confident that she or he is fully competent to undertake them.  This will be clearly recorded.

Additional information:

All calls to this line will be answered by a trained nurse

You will be given information on the services available to you, taking into consideration whether the request to Hospice at Home is for one service only (eg complementary therapy or volunteer service)
