What does it do?

At some point in the future you may be unable to make decisions for yourself about your health or care. We can help you to plan ahead to insure you get the treatment that’s right for you if you cannot speak for yourself.

My Life, My Decision is run in partnership by Compassion in Dying and Age UK Oxfordshire. The project aims to raise awareness of end of life rights and choices, ensuring that you can access information and support to enable you to make informed choices about your end-of-life treatment and to ensure that your decisions will be respected.

Why is planning ahead important?

If you have ideas about what kind of treatment you would or would not want if you didn’t have capacity, then planning ahead will help to ensure your wishes are followed.
 
We can help you record your wishes in a legally binding way. We can support you to make an Advanced Decision, Advanced Statement and a Lasting Power of Attorney for Health and Welfare.

Older people in Oxfordshire.

Local service (South Oxfordshire, Vale of White Horse, West Oxfordshire, Cherwell, Oxford).

The service is free.
