There are times in life where we need help getting things sorted out, in particular during times of change, the death of a loved one or because we are struggling to cope.

A Care Navigator may be able to help you get life back on track, they can talk with you about your needs and concerns and discuss what help you would like to receive.

They can then provide information and options, to help you access other local services and/or provide you with the practical help and support you need.

Help can include but is not limited to:

  • Help with benefits
  • Help choosing and accessing care
  • Help with home maintenance
  • Help understanding housing choices
