Are you or someone you know living with dementia or memory concerns?  Could you benefit from a free service which could retain your independence and enjoyment of life?  Then our service maybe for you. 

The Dementia Community Links support team visit individuals in their own homes and their local community.  We enable service users to engage with community run services such as luncheon clubs, craft activities and exercise groups. 

The team leader will meet with the individual and assist them to design a plan around their needs.  This will enable the support worker and service user to engage in activities which will meet their desired needs.
