What does it do?

Community Links for People with Dementia enabling people with memory concerns to maintain independence and achieve wellbeing through meaningful activity and the pursuit of interests

Dementia Community Links (formerly Flexible Outreach)

Are you or someone you know living with dementia or memory concerns?  Could you benefit from a free service which could retain your independence and enjoyment of life?  Then our service maybe for you.

The Dementia Community Links support team visit individuals in their own homes and their local community.  We enable service users to engage with community run services such as luncheon clubs, craft activities and exercise groups.

 The team leader will meet with the individual and assist them to design a plan around their needs.  This will enable the support worker and service user to engage in activities which will meet their desired needs.

Providing support for individuals with dementia type illnesses in Central Lancashire

Where it is available?

Local service (South Ribble, Preston).

How to access or apply for it:

The service operates in the Preston and South Ribble area and is open to people with dementia or memory concerns who are not accessing Day Care for two days or more per week.  The service is based at Charnley Fold Resource Centre Cottage Lane, Bamber Bridge.  Runs from Monday to Friday 9am to 5pm.  For further information please phone 01772 620876
