Your Life Your Way offer care and support services to individuals in receipt of a direct payment, a personal budget or a personal health budget. We can also work with individuals in receipt of continuing health care funding and self-funders.In some areas we are commissioned by Local Authorities or Clinical Commissioning Groups to offer additional services.

Your Life Your Way delivers services to support you to have choice and control over your care and your life, including care provision to you in your own home, and managed account services for Personal Health Budgets and Direct Payments. We work with you to ensure our services suit you and your needs from beginning to end.

There is a cost but usually tghis is funded by a personal Health Budget that is available to the individual. We can help and advise on the process to secure this, if it is not already in place.
