Going Home from Hospital Care at Home Service

What does it do?

We, in partnership with the NHS, provide care in your home services for up to 12 weeks. Our going home from hospital care at home service wants to make sure that you can go home and live safely.
When you go home, we may give you a few weeks help at home with:
* Dressing and undressing
* Personal hygiene
* Preparing meals
* Encouraging you to take your medication
* Changing TED stockings

Who it is for?

Adults in Newcastle in certain situations who come out of hospital and meet our eligibility criteria for this service.

Where it is available?

Local service (Newcastle upon Tyne).

What it costs:

We do not charge for the Hospital Discharge team.

How to access or apply for it:

If you think you can get help from the Going Home Service, ask to speak to your hospital social worker before you leave hospital.

If you have any questions about the service you can speak to a member of the going home from hospital care at home service team. They are available Mondays to Sundays from 7.30am to 9pm. Phone: 0191 278 2898

What to expect then:

We aim to offer a high standard of service by making sure of:

* Confidentiality
* A flexible and reliable service, 7 days a week from 7.30 am to 10pm
* A visit from one of our staff within 24 hours of leaving hospital
* A review of the care we give you after an agreed time.
